Bertrand Chaffee Hospital’s Primary Care Center treats patients of all ages, and is equipped to see pediatric patients, adult patients and geriatric patients. Our team of physicians, nurse practitioners and physician assistants work with our nursing team and patient care service representatives to be a partner in the management and improvement of your health.

We established Primary Care in our hospital in October 2010 based on the need we saw in our community. Under our care, you can be evaluated, monitored, diagnosed, treated and counseled for your symptoms, conditions or illnesses. As needed, we can refer you to other health professionals, specialists, and community health and social service agencies.

Consider us for your next physical, well child visit or women’s health exam. Our office can handle most routine health concerns related to acute illness, infection and injury.

Bertrand Chaffee Primary Care Center treats a wide range of patients from Pediatric to Adult Care and Geriatrics. We offer preventative care services such as Well Child Visits, Adult Physical Exams and Pap Smears.

Clinical Services

Evaluation – initially approaching a patient of any age group to elicit a detailed and accurate history, perform an appropriate physical examination, delineate problems and record and present data.

Monitoring – providers conduct rounds in acute settings, providing care in office-based care settings, developing and implementing patient management plans and recording progress notes.

Diagnostics – Utilizing common laboratory, radiological, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes which are interpreted by the physicians.

Therapeutics – performing routine procedures such as injections, immunizations, wound care, managing conditions produced by infections or by trauma, illnesses and injuries and performing the evaluation and therapeutic procedures.

Counseling – instructing and counseling patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems or daily living and health maintenance.

Referral – facilitating the referral of patients to other health related professionals and community health and social service agencies when appropriate.
