Rehabilitation Services

Our Rehabilitation Services Program provides a bridge from hospital to home with an interdisciplinary team made up of nurses, physicians, therapists, dietitians, social workers, and activities professionals. Our goal is to assist in quick and safe recovery, back to independent community living, or to an alternate level of care.

The treatment team, along with input from the patient and family, develops innovative approaches to help patients achieve their full potential, while helping to prevent further physical disabilities that could limit future independence. Physical and Occupational Therapists are available seven (7) days a week to provide comprehensive treatment modalities, including strengthening exercises, range of motion, balance training, pain reduction, ambulation, functional training, and patient and family education. A Speech-Language Pathologist is available to address speech, swallowing and cognitive deficits as needed.

Team conference meetings are held regularly to review the Treatment Plan, discuss the patient’s progress, and help to facilitate a safe discharge home.
