Patient Registration

Patient Registration is located next to Coffee Culture in the Main Lobby of Bertrand Chaffee Hospital.

All patients seeking care are required to register prior to reporting to the Outpatient Department for treatment unless prior arrangements are made.

Hours of Operation

Monday - Friday: 6:00am until midnight.

Saturday, Sunday and Holidays: 7:00am - 11:00 pm

What Should a Patient Bring with Them?

Insurance Card;
Prescription indicating service necessary;
Co-pay if applicable;
Any other pertinant information.

To Patients with Standing Order (Routine) Blood Tests:

If you visit our laboratory for a blood test that is done routinely on a monthly or more frequent basis you have the option to pre-register the evening prior to your test. This will enable you to go directly to the lab without having to stop at registration first. Please see guidelines below:

Guidelines for Pre-registration for routine test only:

Call (716) 592-2871 or 1-800-640-8033 then dial "0" for the operator.
The operator will verify your name, date of birth, address etc...
The doctor who ordered the test.
Any insurance changes since your last visit.

Hours for Pre-registration for routine test are:

Monday - Friday between 6:00pm - 8:00pm only.

Sundays between 2:00pm - 5:00pm only.

Please note you will not be able to Pre-register the same day as your test.
